Bio-Medical Waste Segregation Protocols

Life-saving technical protocols for hospitals on color-coding, barcoding, and safe disposal of BMW.

Quick answer

Bio-medical waste must be segregated at source into colour-coded containers as specified in the Bio-Medical Waste Management Rules, 2016, barcoded, stored safely and handed to an authorised common bio-medical waste treatment facility, with daily records, staff training and an annual report to the pollution control board.

1. The Lethal Nature of BMW

Bio-Medical Waste (BMW) represents one of the most hazardous waste streams in the world. Mixed with highly infectious pathogens, expired pharmaceuticals, and sharp surgical instruments, improper disposal can trigger massive community disease outbreaks and fatal needle-stick injuries to sanitation workers.

2. The Universal Color-Coding Mandate

The Bio-Medical Waste Management Rules mandate strict segregation at the exact point of generation (the operation theater, the ward, or the lab). Mixing waste negates the entire treatment process.

  • Y Yellow Category (Incineration)

Human anatomical waste, animal waste, soiled waste (blood-soaked cotton), expired medicines, and chemical liquid waste. This waste is highly infectious and must be incinerated at temperatures exceeding 1050°C to destroy all pathogens.

  • R Red Category (Autoclaving)

Contaminated recyclable waste like IV tubes, catheters, syringes (without needles), and gloves. These are sterilized using high-pressure steam (autoclaving) and then shredded for plastic recycling.

  • W White Category (Sharps)

Waste sharps including needles, scalpels, and blades. Must be placed in puncture-proof, tamper-proof containers to prevent lethal transmission of HIV or Hepatitis B.

3. Barcoding and Tracking

To prevent illegal dumping, all BMW bags must be barcoded before they are handed over to the Common Bio-Medical Waste Treatment Facility (CBMWTF). The barcode traces the bag back to the specific ward of the specific hospital, ensuring 100% accountability.
